When to leave both
  • Fastrack information should only be listed under Patient Note, if the order task is still open.
  • If the patient declines email option (i.e., asked for email, patient declined).
  • Permanent Demographic change details
  • Insurance change details
  • Notes/Important info pertaining to the call. 
  • When talking to a family member, spouse or caregiver – note who you were talking to as an Add Other Task AND a Patient Note.
  • If a patient has a temporary/seasonal address, or secondary phone number they should also be recorded as an Add Other Task as well as a Patient Note
